It is important to note that this information only applies if the Fountain Police Department responded to your accident.

GETTING YOUR TRAFFIC ACCIDENT REPORT IN-PERSON

To make a request in-person, complete a Records Request Form (PDF), found within the official City of Fountain’s Administration Division Police Page.

Complete the form with as much information as possible to help the records department process your request in a timely manner. You will need the following information:

  • Name
  • Phone Number
  • Address
  • Date of Birth
  • Email Address
  • Case Report Number
  • Date of Accident
  • A legible photocopy of your Photo ID

Once the form is complete, bring it to the Fountain Police Department:

The Monument Police Department is open Monday – Friday, 8:00am-5:00pm. You may also pick up the form in-person at the Fountain PD.

GETTING YOUR TRAFFIC ACCIDENT REPORT BY EMAIL, MAIL, OR FAX

You may fill out the Records Request Form online and send it via one of the following ways to request your report:

Other Important Information

  • Records will be released within 3 business days after the request form is submitted.
  • The Fountain Police Department charges about $6 per report. You will receive a phone call to let you know the exact payment required at the time of pickup. Payment must be made before records are released.
  • Be sure to include a working phone number so the records department can contact you if more information is needed. They will call you when the report is ready for pick up.
  • Don’t forget to sign and date the form, the Fountain PD can not process incomplete requests for traffic accident reports.
