It is important to note that this information only applies if the Monument Police Department responded to your accident.

GETTING YOUR TRAFFIC ACCIDENT REPORT IN-PERSON

To make a request in-person, complete a Records Request Form (PDF), found on the Records Release Page on the Monument official website.

Fill out the form with as much information as possible to help the records department process your request in a timely manner. You will need the following information:

  • Name
  • Phone Number
  • Address
  • Date of Birth
  • Email Address
  • Case Number
  • Date of Accident
  • Driver License Number and State
  • A legible photocopy of your Photo ID

Once the form is complete, bring it to the Monument Police Department:

The Monument Police Department is open Monday – Friday, 8:00am-5:00pm. You may also pick up the form in-person at the Monument PD.

Other Important Information

  • Please allow 1-3 business days for report processing.
  • There is a fee of $5 for records up to 10 pages, then $0.25 for each additional page.
  • A copy of your driver’s license will be taken at the time the request is submitted. Please provide a copy of your drivers license when making a request by mail, email or fax.
